Do I Qualify for Home and community-based services (HCBS)?
You may qualify for HCBS if you meet the following criteria:
- You have a chronic/long-term condition. A condition is considered to be long-term if it has lasted for longer than six months.
- You have a monthly income of less than $2,163.
- You own your own home.
- You have less than $8,000 in countable resources or assets. These can include savings, property, bonds, etc.

- Community Health Choices (CHC)
- Private Pay
